We are seeking a dedicated and experienced Care Manager RN to join our client's leading hospital system in Orange, California. In this pivotal role, you will collaborate with a multidisciplinary team to coordinate patient care and manage resources across the healthcare continuum. Your expertise will ensure patient-centered, outcome-driven care that aligns with our commitment to excellence.

Key Responsibilities:

  • Coordinate comprehensive care plans in collaboration with patients, families, physicians, nurses, social workers, and other healthcare professionals.
  • Manage resource utilization to ensure efficient and effective patient care delivery.
  • Facilitate communication among care team members to promote seamless transitions and continuity of care.
  • Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
  • Ensure compliance with all relevant healthcare regulations and standards.

Qualifications:

  • Graduate of an accredited nursing program.
  • Active California RN license (required upon hire).
  • Bachelor’s or Master’s degree in Nursing.
  • Certification in Case Management (CCM or ACM).
  • Specialty certification (e.g., Oncology, Rehab, Pediatrics, Transplant, Wound Care) preferred.
  • Minimum of 3 years of clinical experience in an HMO, medical group, hospital, or clinic setting.
  • At least 3 years of experience in utilization or case management.
  • Minimum of 2 years of recent hospital-based RN experience with adult patients.
  • At least 1 year of direct case management or discharge planning experience in an inpatient setting.
