We are seeking a dedicated and experienced Care Manager RN to join our client's leading hospital system in Orange, California. In this pivotal role, you will collaborate with a multidisciplinary team to coordinate patient care and manage resources across the healthcare continuum. Your expertise will ensure patient-centered, outcome-driven care that aligns with our commitment to excellence.

Key Responsibilities:

Coordinate comprehensive care plans in collaboration with patients, families, physicians, nurses, social workers, and other healthcare professionals.
Manage resource utilization to ensure efficient and effective patient care delivery.
Facilitate communication among care team members to promote seamless transitions and continuity of care.
Monitor patient progress and adjust care plans as necessary to achieve desired health outcomes.
Ensure compliance with all relevant healthcare regulations and standards.

Qualifications:

Graduate of an accredited nursing program.
Active California RN license (required upon hire).
Bachelor’s or Master’s degree in Nursing.
Certification in Case Management (CCM or ACM).
Specialty certification (e.g., Oncology, Rehab, Pediatrics, Transplant, Wound Care) preferred.
Minimum of 3 years of clinical experience in an HMO, medical group, hospital, or clinic setting.
At least 3 years of experience in utilization or case management.
Minimum of 2 years of recent hospital-based RN experience with adult patients.
At least 1 year of direct case management or discharge planning experience in an inpatient setting.

Salary & Benefits:
