Our client, a dynamic health services organization, is actively seeking experienced Physical Therapists to join their team. This vital role focuses on providing both restorative and rehabilitative physical therapy services to participants at the center and in their homes. Collaborating closely with Primary Care Physicians, the rehabilitation team, and other interdisciplinary team (IDT) members, the Physical Therapist will enhance participants' independence, safety, and functional abilities.
Essential Job Duties:
Conduct comprehensive Physical Therapy assessments in facility and home settings to evaluate care needs, preferences, and goals of participants.
Participate in IDT meetings to assist in developing effective care plans.
Implement and document Physical Therapy interventions as directed by physicians and care plans, focusing on exercises, muscle re-education, gait training, and functional training for activities of daily living (ADLs). Utilize modalities such as massage, heat, and electrical stimulation.
Supervise and train Therapy Assistants in implementing Functional Maintenance Programs (FMP).
Educate staff, participants, and caregivers on the correct use and maintenance of therapeutic equipment.
Job Requirements:
A Bachelor's Degree in Physical Therapy is required; a Master's Degree or Doctorate in Physical Therapy is preferred.
Must hold a valid California Physical Therapy License or provide proof of eligibility.
At least two years of clinical experience with frail or elderly populations preferred.
Five years of overall experience as a Physical Therapist.
Current CPR certification is preferred.
If educated outside the US, the Physical Therapy degree must be evaluated as equivalent to US standards.
